Temporary Quarantine Housing Assistance - What do I need to be reimbursed for my hotel stay?
- An affidavit form- attesting to the need to quarantine in a hotel and that no other options were available.
- Positive COVID test result- with your name or a member of your households name
- The Positive COVID test result must coincide with the hotel stay and have your name or household member name
- Negative COVID result with your name or household member.
- Hotel bill/invoice in your name
- Proof of payment- either check clearance or credit card statement.
- Drivers License
