Prescription Drug Program - Does Community & Human Services (CHS) have a prescription drug program?

YES, we do have a prescription drug program where we can provide a voucher for medications at a cost of $5 each.

(This program does not apply towards any type of pain medication)

The patient must have no medical insurance, be under 65 years of age, and has applied for Medicaid and been denied.

Before we can help, they must first apply for Medicaid at: 1 (866) 762-2237 (to file by phone), or go to: Myflorida/Accessflorida (to apply on-line).

They must bring all of the following items at the time of the appointment with CHS - Proof of gross family income for the past 4 weeks; Letter of denial from Medicaid; Most recent bank statement, Proof of Collier County residency; Driver's license or picture ID, Birth Certificate or Passport; and Written Prescription from your Doctor.

Once they receive the denial letter from Medicaid and have all the above items please call: (239) 252-4228 to make an appointment with a Social Services Case Manager.
